Parental Support:

Our Savior Lutheran Chruch identifies the home as the primary place for faith development and strives to align with the Lutheran Chruch Missouri Synod’s 7 Practices for Youth Ministry.  These 7 practices recognize the importance of engaged parents and the integral role they play in the spiritual growth:

• Engaged parents prioritize faith development by encouraging daily faith practices
• Engaged parents invest in the lives of their children by understanding their children’s vocation and the cluture around them.
• Engaged parents participate in worship, personal spiritual practices, and service.

We give thanks to God for your role as parent and the ways you encourage the spiritual growth of your child(ren).

Parent Consent:

I give permission for my son and/or daughter to take part in student ministry activities at Our Savior Lutheran Church (OSLC).  I agree that OSLC will not be held responsible for any injuries or illnesses that my child sustains during OSLC sponsored activities and events.  I declare that my child is covered by primary insurance and assume all responsibility and liability for injury to my child. 

I hereby authorize an adult leader, as an agent of myself, to seek emergency health care, if deemed necessary by said adult leader.  In the event that I cannot be contacted in an emergency, I authorize the physician or hospital selected by the leader to provide treatment, including hospitalization, for my child.

Consent is also given to the photographing of our (my) child(ern) and the recording of his/her voice and the use of these photographs and/or recordings singularly or in conjunction with other photographs and/or recordings for advertising, publicity, commercial or other business purposes.  It is understood that the term “photograph” as used herein encompasses both still photographs and motion picture footage.
